If you want to seek insurance reimbursement we would be happy to provide you an itemized invoice that you can submit to your insurance company. We do not guarantee that your insurance will provide any reimbursement for our services.
- Payment is expected when services are rendered.
- We accept HSA/FSA cards.
- We require 24 hours notice for cancellation to avoid a $50 cancellation fee. If you are more than 10 minutes late your appointment may be canceled, if canceled you will be subject to a $50 fee.
- Having an appointment with Alchemy Health and Wellness does not guarantee that I will be eligible to receive any of the services offered. It is at the medical providers’ discretion to provide treatment.
- There are no refunds for services.
- Alchemy Wellness does not provide primary care services, you will need to continue to follow up with your primary care provider.
